Calcium is the most abundant mineral in the human body, essential for skeletal mineralization, muscle contraction, nerve signaling, and blood clotting. Because 99% of the body’s calcium is stored in bone, supplementation is often treated as a simple “more is better” problem. Physiologically, this is incorrect: calcium absorption is a saturable process, and taking a large dose in one sitting wastes a substantial fraction of it.

Why More Isn’t Better: The Saturable Absorption Ceiling

Intestinal calcium uptake occurs through two parallel routes. Active transcellular transport is vitamin D-dependent and saturable — it dominates when luminal calcium concentration is low and becomes overwhelmed quickly as dose size increases. Passive paracellular diffusion scales with concentration but is comparatively inefficient per milligram.

[!WARNING] In patients with achlorhydria (absent stomach acid), fractional absorption of calcium carbonate has been measured at roughly a tenth of that of calcium citrate under otherwise identical conditions. If you take a proton pump inhibitor (PPI) or H2 blocker, or are over 65, calcium citrate is the more reliable choice — carbonate depends entirely on gastric acid it may not have.

The Critical Drug Interactions

Levothyroxine (Thyroid Medication)

Calcium carbonate binds directly to levothyroxine in the gut, forming an insoluble complex. In a controlled crossover study, co-administering 2 g of calcium with levothyroxine reduced T4 absorption from roughly 84% down to 58% of the dose.

[!CAUTION] Levothyroxine and calcium (in any form, including fortified foods) must be separated by at least 4 hours. This mirrors the same interaction iron has with thyroid medication — if you take levothyroxine, plan your entire mineral stack around it, not the other way around.

Bisphosphonates (Osteoporosis Medication)

This is the most ironic interaction in supplementation: bisphosphonates and calcium are frequently prescribed together for bone health, yet calcium is exactly what destroys the drug’s bioavailability. Bisphosphonates form stable, insoluble chelate complexes with calcium and other divalent cations, and food or supplements containing calcium taken too close to a dose can reduce absorption to near zero.

[!WARNING] Take bisphosphonates first thing in the morning on a completely empty stomach with plain water only, and wait at least 30–60 minutes before any food, coffee, or calcium supplement.

Iron and Zinc

Calcium is a potent competitive inhibitor of both heme and non-heme iron absorption at the shared intestinal transport step, and it competes with zinc through overlapping divalent-cation pathways. Never take a calcium supplement in the same sitting as an iron or zinc dose — separate them by at least 2 hours.

The Cardiovascular Question

Calcium supplementation is not risk-free at the population level. A large meta-analysis of randomized, placebo-controlled trials found that calcium supplements taken without co-administered vitamin D were associated with an increased risk of myocardial infarction — on the order of a 30% relative increase in the pooled trial data. The mechanism is thought to relate to the sharp, unphysiological spike in serum calcium that follows a large supplemental bolus, which dietary calcium (absorbed gradually with food) does not produce.

[!TIP] This is not a reason to avoid calcium — it’s a reason to prefer food sources where possible, and when supplementing, to keep single doses small (≤500 mg), split across the day, and paired with vitamin D rather than taken as an isolated high-dose bolus.
